For All Requests, include:
- Email or Written Statement for the Student records you need (Attendance, Immunizations, Proof of Enrollment) with Student Info:
- Student Name (During Enrollment)
- Student DOB
- Last School Attended
- How would you like the records send? Email, Mail, or Pick-up?
- Your Contact Info, Phone number or Email.
- Scanned copy of a Valid Government-issued ID.
*Email is the fastest way to request/receive records*
Note: When a student is over 18 years of age, no person other than the student may receive their records, including education verification, without the adult student's written authorization.
The district has five business days to provide the transcript or records pursuant to Section 49069 of the California Ed Code.
