REPORTING A WORK-RELATED INJURY
When an employee is injured at work, follow these instructions:
In an EMERGENCY:
- EMERGENCY: If a life-threatening injury or accident occurs, Call 911 or go to the nearest hospital for immediate assessment/treatment.
- As soon as possible, please complete the next 3 items:
- Report the injury to your supervisor.
- Call and report injury to the Early Intervention Nurse at 1-877-742-3467.
- Copies of emergency room documents and bills will need to be submitted to School's Insurance Authority (SIA)
- Employee/Supervisors will need to submit an incident report form to the Site Administrator or Director to review and submit to the Coordinator of Risk Management within 24 hours of the injury or accident.
- Email a copy or photo of the Industrial Work Status Report (IWSR) to workerscompensation@husd.k12.ca.us for review.
For NON-EMERGENCY injuries:
- Immediately report the injury or accident to your supervisor
- Call the Early Intervention Nurse at 1-877-742-3467 to report injury and seek treatment
- Employee/Supervisors will need to submit an incident report form to the Site Administrator or Director to review and submit to the Coordinator of Risk Management within 24 hours of the injury or accident.
Please note: In the event of non-emergencies, seeking medical treatment before reporting the injury to your supervisor and SIA's Early Intervention Nurse may be considered self- procured medical treatment and the District may not be responsible for payment.
Open Worker's Compensation Claim
Once an injury gets reported to the Early Intervention Nurse at 1-877-742-3467, a Worker's Compensation Claim will be opened for that particular injury with School's Insurance Authority (SIA). The Early Intervention Nurse will conduct the initial intake, and direct the employee as needed.
Anytime you visit the doctor due to this injury, you will be given an Industrial Work Status Report (IWSR) that will list any work restrictions or modified activities assessed by the doctor. Please email a copy (or photo) of the IWSR to workerscompensation@husd.k12.ca.us for the Workplace Health and Safety Manager to review. After reviewing your work restrictions or modified activities, the Workplace Health and Safety Manager will let you and your supervisor know if you are cleared to return to work, or if you should remain off work.
