Personal Property Loss - HUSD Employees
Upon the loss of property, as referenced in Contract Bargaining Agreements (ACSA: Article 3100, AEOTE: Article 21, Letter A, HEA: Article 9, Letter F, SEIU SAFECO: Article 21, Letter F, SEIU PARA/YEP: Article 20, Letter C) complete the following:
- File an Incident Report
- Proof of estimate or proof of value of item
- Personal Property Loss Reimbursement Form
The following information is to be included:
- Employee Name
- Incident Date / Time
- Location
- Amount of Loss
- Date of Incident Report Completed
- Description of how the loss occurred
